Executive Summary

The pandemic has wreaked havoc with health care systems’ surgical staffs. Some clinicians are fatigued and stressed, and many are out of practice. They are having to contend with large backlogs of procedures that were postponed. And they have to contend with the ongoing pandemic, with all its uncertainties. To keep surgical staffs and patients safe during these difficult times, health systems should take four steps: make risk visible; ensure that staffs are abiding by existing procedures and protocols and adopting new ones when the need arises; double down…

Unwarranted variations. Among health care systems and payers—both government and private—this term implies unnecessary risk to patients, diminished quality of care, and increased health care costs. From the numerous studies and papers published on the topic, a common description emerges: unwarranted variations are not directly due to the inherent variations of a given patient; rather to the variations in the processes and conditions of how care is delivered, especially when it results in inconsistencies in the adherence to and administering of evidence-based guidelines of…

Covid-19 has exposed vulnerabilities in health care systems across the United States and world. To reduce infectious risk to patients and providers, and conserve critical resources — such as personal protective equipment (PPE), ventilators, and intensive care (ICU) beds — most states in the U.S. enacted a temporary ban on elective surgery from March through May 2020.

The ban has resulted in a backlog of uncompleted procedures that had been scheduled over this three-month period, as well as a dynamic backlog of surgeries that continue…
