This form contains 29 questions to do with the identification of a company's vulnerabilities in a number of areas: goals, work, policy, monitoring and training.
This template is intended for managers who will analyze the answers given and then determine how clear the company's goals are, how competent its employees are in different areas (their compliance with rules and regulations, the effectiveness of their internal and external communications etc.), which moral and ethical codes are upheld by the team, plus how and when internal assessments and audits are carried out.
This template should be filled out on a periodic basis to ensure continuous assessment and identification of weak areas in the company's management and general activities. After analyzing the results, managers should make decisions on how to improve the situation in one area or another (whether there's a weak culture or poor competency). That is to say, managers need to allocate resources for the coordination of employees' work, the running of seminars and the optimization of business processes.
