An example of a scorecard for use in seminars or work meetings on risk evaluation. The template is utilized as the simplest means of evaluating risks, so it's suitable for companies just starting to manage their risks.

The template includes risk evaluation criteria, specifically to do with the level of damage incurred as a result of a risk occurring and the likelihood of such a risk happening over a given period of time (for example, a year or quarter). Each criterion is rated from minimal to maximal significance. For the construction of a risk chart, it uses qualitative evaluation (for example, risk 1: low damage - high likelihood, and other combinations for each risk identified). The scorecard allows for the creation of a risk chart, which is a graphical description of risks facing an organization in the form of a table.

The scorecard template allows managers and their coworkers to use the results of their work meetings to analyze and evaluate risks facing the company and then create a risk chart. In turn, this chart allows for the measurement of each occurring risk and thus informs subsequent decision making regarding its further analysis or management.

A template is filled out by each employee so that the completed templates can be aggregated. It will not completely solve problems associated with the subjective evaluation of risks.
