This is a short questionnaire which allows a firm's leadership to determine the readiness of individual employees to manage risk within the enterprise. The results of this survey allow for the identification of common values, acceptable forms of conduct, and a system of incentives enabling employees to take risks into account while working.

The use of this questionnaire allows managers to assess the maturity of risk management culture within the company. The development of a risk management culture means that employees are able to identify risks in a timely manner, as well as to analyze and mitigate these risks.

It is recommended that a survey be conducted among employees at least once a year. Filling out this questionnaire is not obligatory, but it is recommended if you want to strengthen risk management culture within your company.
